The process for obtaining school transcripts for current students or graduates.

Transcript Request Procedures (Lewis High School)

Current Students

Transcripts are the school records that document a student’s attendance, classes taken, and grades earned during each school year.

A request for a student's transcript (or other school records) requires a signed release:

  • from the student, if he or she is 18 years of age or older or is enrolled in postsecondary education
  • from the parent or legal guardian, if the student is under 18 years of age.

To request an official transcript, complete the Current Student Transcript Request Form and Consent for Release Form. These forms can be emailed to Zarima Ayubi at [email protected] or brought to her office in Student Services, Room 34.

Former Students Who Graduated Within the Last Five Years

Former students of Lewis HS should complete the Transcript Request Form and Consent for Release Form and then submit it electronically to Ms. Zarima Ayubi at [email protected] along with a copy of a photo ID. Electronic payment for transcripts can be made at MySchoolBucks.  Requests and payments can also be made in person at the school.

Once all payment and all documents are received, the request will be processed within 48 business hours.
