Below you will find the forms you will need to start the new school year. This year completion of forms to return to school will be completed online due to COVID -19. Please complete all of the required forms and email them by Friday, August 28th.

Required Forms for All Students

Emergency Care Form

• All students must have accurate emergency care information. Parents update this information through @email via their SIS ParentVue account. 

• Log into your SIS ParentVUE account 

• Go to FCPS Links located in the left-hand menu
• Go to FCPS 24/7 and Log in using the same username and password as your SIS ParentVUE account
• Go to WeCare @ School on the top right of your screen.
• In order to log into Parent View it is necessary to have an active SIS Parent Account - if you need a SIS Parent Account, please contact Lewis HS Registrar Katherine Kidd at [email protected] or you can call at 703-924-8322"

Health Information Form

• Complete the Health Information Form by Friday, August 28th.  You can email this form to Katherine Kidd at [email protected] or 703-924-8322"

Military Connected Students (Required for all students)

In accordance with the Code of Virginia (§22.1-287.04), local school divisions are required to identify students who have a parent in the United States uniformed services. Completing this form allows Virginia localities to maintain reliable and accurate data for potential grant funding and to receive services to meet the needs of uniformed services- connected students.

Optional Forms

FCPS Privacy and Opt-Out Forms

• Details parents' rights regarding FCPS policies. Complete these forms only if you wish for your child to be opted-out. Please submit by Friday, August 28th.

Family Life Education Opt-Out Forms

• Information about Family Life Education can be found on the FCPS website for Grades 9-12. Please submit by Friday, August 28th.

Free and Reduced Lunch Application

• Apply for Free and Reduced Lunch or view nformation about the FCPS Food and Nutrition Services. 

Homeless Student Referral Form

• If applicable, please complete this form and email to Ms. Marcella Fulmore at MYFulmore@fcps.edu.

SIS ParentVUE

• SIS ParentVUE provides online access to information about your child’s attendance, class performance, demographic data and course materials.  There is a SIS ParentVUE Mobile App available.

• All parents will use their SIS account to update emergency care information for their children.

• Parents of returning students can reset their SIS ParentVUE password, change their username or recover their username by clicking on the appropriate link.

• Parents will be invoiced for the $50 technology support fee in October 2020 through MySchoolBucks.com.

MySchoolBucks

• Parents can use this online payment system to check balances and add money to school cafeteria accounts, to make payments to the school for various school related items and to receive low account balance notifications.

• Visit our athletic site for for information including teams, game schedules, Booster Memberships, spirit wear, and more.

Yearbooks

• Yearbook ordering information will be available in September.
